Phase 2 live since 21 Sep 2026; entering 10-week stabilisation and outcome capture window. 4 KPIs tracked (LOS, 72h re-attendance, serious adverse events, discharge complaints). Goal: 20% LOS reduction validated by 30 Nov 2026.

Start of new Bulletin trial for one week from today (24 Aug - 31 Aug 2026). Replaced Word document → Excel transcription workflow. Now entering daily bulletins into M365 List, showcased in SharePoint. Removed the transcription step and consolidated all information into SharePoint.

Briefing started for the NC and ANC. Target to roll out in Sep 2026. On track.

Faizuan briefed the NC/ANC last week; on track to start the workshop in Sept 2026.

Met with Synapxe team. They advised benchmarking against similar work done by NGEMR and EP teams, and recommended applying for the green lane to trial the new care delivery model in parallel. Project is now blocked pending IT clearance to proceed — no further implementation work can advance until clearance is granted. Next: follow up on IT clearance and identify NGEMR/EP contacts for benchmarking.

Project charter drafted and saved as MASTER.md (covers 4-role team structure, AIC command-and-control, EPIC Secure Chat workflow, EPIC ASAP dashboard, medication optimisation, AI integration via Pair.gov.sg + AI Chat Agent, PDSA methodology, 4-phase implementation timeline, and 3 KPI categories). MASTER.md structured for direct NotebookLM upload. Tracker record created; SMART goal, co-leaders and members still pending — [INCOMPLETE SMART — needs S/M/T]. Next step: confirm SMART goal + leadership line with Gay Cheong.

Signage arrived from facilities; SSN/ANC briefed on station use; KPI dashboard mounted on north wall. Awaiting end-of-week compliance audit.

SSN/ANC station location finalized (north junction). KPI dashboard draft complete. Pending: signage purchase, staff briefing.

Mapped current P2 layout and shift roster; ran a 2-week time-and-motion sample of SSN/ANC time allocation; drafted first-pass area map.

Strengthening Competence and Confidence Through Structured Learning Interventions for Junior Resuscitation Nurses

Project initiated 15 Jul by Leo. Source: 14 Jul ED Operations Improvement meeting — decision to use PRISM as central near-miss DB; no penalisation for near-miss reporting; separate monthly reports for leaders and ground staff. Owner: Teik Sum (medication lead). Cross-team with P2 redesign; Leo as co-lead. SMART goal still TBD with Teik Sum.

Hand hygiene compliance targets achieved: above 70% at hospital level and above 80% at department level. Project goal met.

Project opened in response to 20% compliance audit (all-time department low). Hidayah operational lead, Leo supervisor. Tactical plan (week 1) + strategic plan (12-week horizon, target >=70% by 30 Sep).

Project closed: NLD rollout completed on schedule. Phase 1 (7-20 Sep, NCIC-led) finished; Phase 2 (ground-nurse-led) live from 21 Sep. Stabilisation and outcome review continues as new project #10.

Phase 1 completed (7-20 Sep, NCIC-led). Phase 2 now underway: ground nurses independently managing NLD workflow from 21 Sep 2026 onwards.

NLD rollout timeline confirmed: 1 Sep - Start of pre-go-live staff huddles; 7 Sep - EDTU Nurse-Led Discharge Go-Live; 7 to 20 Sep - Support and stabilisation period; 21 Sep - Full implementation for ground nurses. Project goal: smooth transition, confident teams, excellent patient care.

Hidayah and Reena worked out the checklist and KPI tracking list; on track to start the 2nd week of Sept 2026.

Project created. Direction noted: value-based care oriented. Awaiting full charter from Leo (problem statement, SMART goal, value-based-care outcomes, scope definition, timeline).
